History of Present Illness: - Asymptomatic
Medical History: - 22 years old

- Never hospitalized

- No current medical problems

- Currently taking oral contraceptive

Psychosocial History: - Single

- No children

- Watches calories

- Performs aerobic exercise 1 to 2 times per week

Physical Exam Findings: - Height: 5'5"

- Weight: 130 lbs.

- BP: 125/80

- Pulse: 65 and regular

Smoking Information: - Has smoked 20 to 25 cigarettes per day for 5 years

- Smokes 1 to 2 hours after waking

- Tried to stop 1 year ago but relapsed after gaining 7 lbs.

- Starting new job in 2 weeks in a smoke-free office

- Would like to quit; has limited confidence in her ability to quit
